أمل كبير.. علاج شافٍ لفقر الدم المنجلي خلال 3 أشهر

أكد مدير برنامج زمالة أمراض الدم بشبكة القطيف الصحي الدكتور عبدالله آل زايد، أن هناك أملاً كبيراً في الوصول لعلاج شافٍ لفقر الدم المنجلي في غضون أقل من 90 يوماً.
وأوضح أن الدراسات أثبتت فعالية العلاج الجيني لفقر الدم المنجلي، حيث اختفت نوبات الألم في المرضى الذين استخدموه، مشيرًا إلى أن العلاج الجيني يستخدم حالياً للمرضى من عمر 12 سنة فما فوق، ومن المبكر معرفة مضاعفات العلاج على المدى البعيد.
وأشار د. آل زايد إلى أن تقنية كريسبر وتقنية كريسبر - كاس9 - تعتبر إحدى أهم التطورات العلمية في القرن الحالي، وقد أدت إلى تطوير طريقة حديثة للتحرير الجيني، تبشر بعلاج نهائي لفقر الدم المنجلي.
مرض وراثيولفت إلى أن فقر الدم المنجلي هو مرض وراثي يصيب خلايا الدم الحمراء، مما يؤدي إلى نوبات ألم شديدة والعديد من المضاعفات الأخرى.دكتور عبدالله آل زايد
وقال إن هناك أكثر من 30 علاجًا قيد الدراسة حاليًا، وأن أفضل العلاجات المتاحة حاليًا هو علاج الهيدروكسي يوريا، وهو علاج فعال جدًّا، وله أكثر من خمسة وعشرين عامًا يستخدم لهذا المرض.
وأشار إلى أن هناك ثلاثة علاجات حديثة ظهرت في السنوات الأخيرة، هي: فوكسلوتور «الاسم التجاري أوكسبيرتا Oxbryta» وإل - غلوتامين «L-gutamine والاسم التجاري إنداري Endari».
وأضاف آل زايد: ”كل هذه العلاجات الهدف منها السيطرة على المرض، وليس الشفاء منه. وتبقى زراعة الخلايا الجذعية، أو زراعة نخاع العظم، هي العلاج الوحيد المعتمد للشفاء من المرض“.
وتابع أن «هناك العلاج الجيني الذي يعتبر أحد العلاجات الواعدة لفقر الدم المنجلي، ونحن على وشك الوصول للحظة تاريخية لهذا العلاج فبالتأكيد نحن أقرب إليه بكثير مما كنا عليه في أي وقت مضى».
أول بحث بالمملكةوأكد أن أول تقرير تم نشره عن فقر الدم المنجلي في المملكة كان في سنة 1963م، وذكر التقرير أن أغلب الموظفين المصابين كانوا من واحتيّ القطيف والأحساء.
وأوضح أن هناك فرضيتَين في تفسير انتشار فقر الدم المنجلي في دول الخليج، إذ يعتقد بعض العلماء أن سمة الأنيميا المنجلية ظهرت إما نتيجة طفرة جينية، حدثت بسبب طفيل الملاريا، إذ كانت المنطقة موبوءة بالملاريا وقتها، أو بسبب الهجرات التي كانت تحدث بين الهند ودول الخليج، وكذلك هجرات الأفارقة وجنوب المملكة.
وأشار إلى أن النوع المنتشر في المنطقة الشرقية هو النوع نفسه الموجود في شبه القارة الهندية، بينما النوع الموجود في جنوب المملكة هو النوع نفسه الموجود في إفريقيا. ويلاحظ أن مرضى الشرقية لديهم سمة ثلاسيميا ألفا أكثر من مرضى الجنوب.
وبين أن مستوى الهيموجلوبين الكلي والهيموجلوبين الجنيني أعلى في مرضى الشرقية ومرضى دول الخليج، وهذا ما يجعل شدة المرض تبدو أخف ظاهريًّا، ويتأخر ظهور المضاعفات في مرضى الشرقية - نوعًا ما - مقارنة بمرضى مناطق جنوب الجزيرة العربية.
وأكد أن كلا النوعين يشترك في أغلب الأعراض والمضاعفات، وإن تفاوتت النسب بينهما من حيث الانتشار والشدة.
